چکیده

t h c c Background. Postoperative stays after coronary artery ypass graft surgery (CABG) decreased substantially in he 1990s. Although shorter stays offer clinical benefits, remature discharge could increase adverse events and ffset initial savings. This study examined the effect of arly discharge after CABG on readmission/death and ost within 60 days of discharge home. Variability in ospitals’ tendencies for early discharge and adverse utcomes was also explored. Methods. Analyses were based on clinical and claims ata for 55,889 New York CABG patients discharged ome 1995 to 1998. Early discharge was defined as a ostoperative stay below the 15th percentile for patients ith similar risk. The likelihood of early discharge and ts effect on readmission/death were examined using ierarchical logistic regression, accounting for patient isk and within-hospital correlation. The correlation beween early discharge and adverse outcomes at the hosital level was assessed. The effect of early discharge on

Trials comparing early vs late extubation following cardiovascular surgery.

We identified 10 randomized trials that compared alternative management approaches to patient care during and following cardiovascular surgery. One overall strategy involved a modification of anesthesia, in particular, a reduction in the dosage of fentanyl and benzodiazepine or the substitution of fentanyl for propofol (five randomized controlled trials [RCTs]). Pooled results show a shorter du...
